This patient is a 21 year old college student who came to my Brookline office to discuss breast augmentation in Boston. She has never been pregnant; however, she has lost approximately sixty pounds which has left her with a scooped out appearance of her upper breasts. She wanted to fill in the upper portion of her breasts and also enhance her breast size in order to balance her hips.
 
When we met in my office to discuss plastic surgery we spent a great deal of time discussing her soft tissue envelope and the fact that her breasts are quite widely spaced apart. Breast augmentation surgery will not narrow the distance between the nipples; the breast implants have to be centered underneath the nipples. This patient required a fairly large implant in order to fill her inelastic soft tissue envelope. She decided on an augmentation of just over 400cc and I selected Mentor’s widest saline implant.
 
At surgery the implants were placed under the chest muscle using an inframammary incision. Each implant was filled to 415cc. The incision underneath the breast allows for excellent control of the implant pocket and in patients with a full lower breast, such as this patient, the incision is well concealed. She is very pleased with the youthful appearance of her breasts following her Boston cosmetic surgery.
